What Is a Lip-Flip?

A lip flip with Botox is a non-surgical cosmetic procedure that enhances the look of your upper lip. Unlike volume-enhancing fillers, Botox relaxes the orbicularis oris muscle (the tiny muscles around your mouth/the kissing muscle that helps you pucker and close your lips). It flips the upper lip upward to create a fuller appearance. Ideal candidates include:

How Does Botox Work on Lips?

Because we’re working with such a small, precise amount of Botox in this area, a lip flip is considered one of the most subtle Botox treatments we offer.

We use tiny, precise Botox injections along the border of your upper lip to target the small muscles that control lip movement. The Botox relaxes these muscles just enough so they don’t pull your top lip down as much. With that extra pull reduced, your upper lip can gently roll outward, showing a bit more of your natural lip.

How Many Units of Botox Do You Actually Need for Lip-Flip?

Most of our lip flips fall into a low-dose range, but there isn’t a one-size-fits-all number. The amount we use depends on your lip structure, how strongly your upper lip pulls when you talk or smile, your goals (very subtle vs more noticeable flip), and your previous Botox history. As a guide, we might start a cautious first-time patient at a very light dose of 4 units and then build up at future visits if you want more of a flip. In practice, this still adds up to just a few units of Botox in total, much less than we use for areas like the forehead or frown lines.

Someone with a stronger upper-lip muscle or a gummy smile usually needs a bit more product, carefully divided among the injection points to balance movement and shape. Too little can make the result underwhelming, while too much can temporarily make sipping through a straw, whistling, or pronouncing certain sounds feel different, which is why we always err on the conservative side and adjust gradually.

Many patients are surprised by how little Botox a lip flip actually uses compared to other areas. Smoothing frown lines between the brows or treating horizontal forehead lines can take several times more units than a lip flip does, and crow’s feet at the corners of the eyes also typically require more product overall. When you see it in this context, a lip flip is a very low-dose, highly targeted treatment.

How Is Botox Lip-Flip Different From Lip Fillers?

Although both treatments can enhance the appearance of your lips, they work in different ways.

A lip flip uses small doses of Botox to relax the muscles around the top lip. As these muscles relax, the lip rolls smoothly outward, making more of it visible and creating the appearance of a more voluminous lip without adding fillers.

Lip fillers work differently. We place a hyaluronic acid-based gel into the lips to add physical volume and shape. Hyaluronic acid occurs naturally in the body and helps attract moisture, giving the lips a plumper appearance. Fillers can address concerns such as thin lips, volume loss, uneven lip shape, or a lack of definition along the lip border.

What Happens During the Botox Injection Procedure for a Lip-Flip?

While any cosmetic procedure can sound daunting, the good news is that a lip flip is painless and requires almost no downtime after treatment. Patients are typically in and out of their appointment within 10 to 20 minutes, allowing you to continue with your day as normal.

Before the Injection Procedure

Before starting your procedure, we’ll examine your lip structure and movement patterns to determine the prime injection areas. We’ll then cleanse the skin to prevent infections. Often, first-time patients request a topical numbing cream. While this isn’t necessary, we understand pre-treatment anxiety, especially for newbies.

As a precaution, we evaluate your medical history and understand any pre-existing allergies to minimize the risk of adverse reactions. Dissolving any previous injectables helps us create a predictable foundation for effective treatment. Due to the absence of clinical studies in pregnant women, we do not perform treatments during pregnancy.

While you require no special preparation, we advise our clients to avoid alcohol, blood thinners, and supplements for 48 hours before an appointment. We recommend delaying any dental work for at least 2 weeks after your treatment, as the area might be sensitive and cause discomfort.

During the Injection Procedure

Using an ultra-fine needle, usually 0.2mm wide, we place 4–6 injections along the upper lip border. The total dose is only a small number of units, divided between these points so that each injection does a very specific job in shaping how your upper lip rests and flips when you smile. Our clients refer to it as a brief pinching sensation lasting less than 5 minutes. On average, we complete the procedure in 20 minutes, including prep work and post-treatment care. Many of our clients schedule their lip flip during their lunch break and return to work straight afterward.

After the Injection Procedure

Following your lip flip, you may experience slight redness or swelling in the area; this is completely normal and should resolve within an hour or two. Some patients experience tightness around the mouth; this too will ease within the hour.

The first 4 hours are critical. While the neurotoxin binds to the nerve, you cannot touch the area; this is non-negotiable. Pressure on the injection site may displace the Botox before the binding process is complete.

Do not lie down for at least 2 hours post-treatment. Gravity may cause the neurotoxin to migrate downwards away from the injection sites. Avoid applying any makeup to the lip area for at least four hours after your treatment. The pressure, along with adding product to an area containing micro-punctures, may cause irritation or infection.

Strenuous exercise is another no-go for 24 hours after your lip flip. The increased blood flow may diffuse the neurotoxin, potentially increasing the risk of bruising in adjacent areas. You’ll notice changes between days 5 and 7 after your treatment; your lip begins to flip outwards, creating a fuller appearance. Botox typically takes full effect by day 14, with results lasting up to 3 months.
